1. This parental consent form must be filled out, signed and returned by a parent or guardian.

2. If you would like to include a picture or two, please email it/them to pictures@kidsareheroes.org

3. Fill out the form below and click the "Go" button! Please recheck the spelling of your email address to make sure we can contact you.

Help us go global! If your native language is NOT English, tell us about your hero in your native tongue. We will still need an English translation so please make sure you fill in that field as well. We will showcase your hero in YOUR language with a button to translate so others can read it too!
